Case Manager Lorain County - OH Qualified candidates will be RN, LSW, LISW. Must have 12 months experience in home and community based services within the past 5 years. Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs desirable. It is preferred that Case Manager reside within the counties of coverage.
Case Manager will assess, authorize and provide ongoing case management for a diverse population of persons with chronic illnesses or disabilities of all types. Will work independently, managing a caseload of Ohio Home Care consumers.

| | Provider Monitor - Region 2 Ashland, Crawford, Knox, Marion, Morrow, Richland, Wynadot and surrounding counties - OH The Structural Review (SR) Provider Monitor is responsible for conducting reviews of Waiver providers to assess performance and compliance in accordance with OAC rules.
Qualifications:
Ideal candidates will possess the following qualities:
Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse; Free from specified offenses as evidenced by a criminal background investigation; 12 months experience in Home and Community Based Services Environment; Knowledge of and experience with interpreting and enforcing OAC Waiver rules preferred; Excellent verbal and written communication skills; strong Microsoft Word and Excel skills, strong organizational practices.
Responsibilities:
The Provider Monitor will: Determine provider eligibility for the Structural Review; Conduct face-to-face, desk and/or billing structural reviews of all specified providers; Determine (un)acceptability of all Plans of Correction resulting from non-compliance findings and proceed with additional actions as specified; Make referrals to ODJFS based upon SR findings; Participate in provider education classes specific to the OAC rules regarding provider performance and billing practices; Provide technical assistance to providers as necessary and requested; Participate in process improvement initiatives.
| | QI Assistant Columbus - OH There is an immediate opening for full time QI Assistant. This office based hourly position located on the east side of Columbus is responsible for the implementation protocols to assess and facilitate contract compliance and adherence to quality performance standards. Individual will research documentation, collect and analyze data on quality indicators, examine guardianship documentation against compliance standards, maintains the schedule and logs for After Hours personnel and provide back up coverage for Customer Service representatives as outlined in their position description. Qualified candidate will have a minimum of a high school education (or GED). Have at least 1 yr of solid experience using Microsoft Word, Excel and with use of Power Point as a plus. Demonstrate excellent verbal and written communication skills, exercise independent thinking with attention to detail and present a positive customer service and team oriented outlook.
