Case
Manager
RN, LSW, or LISW
Assess, authorize
services, and provide on-going case management for a
diverse population of persons with chronic illnesses
or disabilities of all type. Will work independently
managing a caseload of Ohio Home Care consumers.
Qualified candidates will be a RN, LSW, or LISW and
must have 12 mo. home or community based experience
within the past 5 years. Knowledge of Medicare/Medicaid,
home health care and waiver programs desirable.
Quality
Improvement Assistant - Part Time
Immediate
opening for a Part Time (flexible hours) Quality Improvement
Assistant. The Quality Improvement Assistant provides
clerical and operational duties that support the functions
of the QI Department. Qualifications
include one year experience in data entry: Experience
creating and using documents in Microsoft Word, Excel,
and Access: Ability to manage time/prioritize projects:
Good communication skills and professional telephone
manner; Ability to work independently under supervision.
Provider
Monitoring Supervisor - Full Time
Full time Provider Monitoring Supervisor needed
to supervise Region 3 staff in the functions related
to the oversight and monitoring of the compliance &
performance of Ohio Home Care Program providers. LSW,
LISW, or RN with at least 24 months exp in home/comm.
based service environment required. Knowledge of Medicare/Medicaid,
home health care waiver programs, and consumer incident
reporting systems desirable. 1 year Supervisory experience
preferred.
Clerical
Assistant - Full Time
Immediate
opening for a full time Clerical Assistant to perform
a variety of clerical support functions. These
functions may include a combination of responsibility
for all filing, processing consumer documents, managing
mail and fax documents, performing tasks within the
CareStar Database and other general administrative/clerical
support tasks.
Web Application Developer
Ideal candidate will have a minimum of 3 years experience developing database-driven, dynamic web sites. The primary technology being utilized is Classic ASP. with .NET experience being a plus. Strong MS SQL application development experience is a must. Other technologies being used include client-side Web development (JavaScript, CSS), AJAX and SQL DTS Scripting.
