Solutions for Home and Community-Based Services (HCBS)

CareStar works with clients to help them meet the objectives of their home and community-based waiver programs. We provide innovative, quality focused solutions that help them improve the care of their consumers in a cost-effective manner.

Case Management
Our case management is defined as a collaborative process that assesses plans, implements, coordinates, monitors and evaluates options and services to meet consumers’ community care needs. We do this through communication and utilization of available resources to promote quality and cost-effective outcomes.

Provider Management
We work with providers to assure that they are enrolled in the program properly and that they are compliant with state regulations. In addition, we track and follow up on any occurrences or incidents.

Quality Assurance and Improvement
We manage the assessment and conduct reviews of clinical and provider services to ensure program compliance. This includes: health care review, outcomes, assessments and evaluations. Our quality is measurable. We are pro-active in the assurance of quality standards.

Information Systems
Information plays a major role in providing the answers and insights to successfully manage the day-to-day activities of a home and community-based service (HCBS) program. Our systems include integrated intranet and internet systems that capture and manage data that helps clients track consumer activity.

Training and Education
Supporting our HCBS efforts is a fully developed and integrated training and education system. This includes: classroom training, seminars, teleconferences and on-line CEU training.

 Waiver programs are growing at nearly double digit rates annually since signed into law in 1981. Consumer demand for services is rising. The way services are offered is also changing as is new Federal and State legislation. Funding is becoming increasingly tight.
