CareStar Case Management
CareStar’s case management model follows the Case Management Society of America’s definition: “A collaborative process that assesses plans, implements, coordinates, monitors and evaluates options and services to meet consumers’ community care needs.” CareStar’s array of services successfully accomplishes this objective while promoting quality and cost-effective outcomes for all of its home and community-based (HCBS) customers.
CareStar's service model is designed to be scalable and capable of managing any number of consumers, regardless of the size and/or complexity of the program. CareStar case managers are highly trained and receive on-going education in the management of chronic consumer diseases, the understanding of current issues in home and community-based care and services and the effective utilization of program resources. CareStar combines professional staff dedication, patient-centered care and state-of-the-art technologies to achieve cost-effective outcomes and improve our consumers' quality of life.
