Your Case Manager

CareStar employs the most qualified professionals to function in the role of a Case Manager. Hiring only RN's and licensed social workers with extensive experience in home and community based services, our professional staff are dedicated to serving the consumer in multiple ways to assure independence through community based care.

What does the case manager do for the consumer?

  • Visits applicants in their home to: 1) gather information and medically assess if the individuals are eligible for home care services per program criteria; and 2) determine if applicants can best be served safely in their home and community.
  • Assists applicants who are approved for services plan for their care by arranging authorized providers to meet the assessed needs and medically necessary services.
  • Coordinates overall service provision with all involved or potential providers and systems such as local schools, developmental disability boards, mental health, or other community resources.
  • Assures consumer health and safety through a careful evaluation of the consumer's living arrangment.
  • Maintains regular contact with consumers and providers to make sure consumer needs are being met; annually reassess participants eligibility for these medically necessary services.
  • Troubleshoots problems for consumers.
  • Documents all activities and communications to assure comprehensive care is monitored and the coordinated care is fulfilling the program's goals and objectives.
