Waiver Case Management
CareStar provides its customers with expertise derived from years of experience in the administrative case management of private and government-funded HCBS programs. Operational in multiple states, CareStar is the sole contracted entity which provides case management for the Ohio Home Care program (OHCP), providing care for over 13,000 participants.
Using its trend-setting case management software, CareStar cost-effectively manages waiver and other HCBS program funds by combining a person-centered approach with highly trained community-based professionals, state-of-the-art technology and a vital network of qualified providers to produce successful outcomes.
Personalized Disease Management Portals
CareStar's newest advancement is a secure, web-based, customized portal system. This portal system is a simple and highly effective tool designed to allow program participants (e.g. consumers, providers and program administrators) to network and access important program information all on one private, individualized web page.
Portals facilitate more effective team communication and can also be used as a platform to provide personalized disease management information and tracking. They also empower our consumers to be better informed in planning and coordinating their own care and services through specialized resources and links which provide care plans, forms, tools, surveys, news/updates, contacts and various information for health improvement.
