Ohio HOME Choice

HELPING OHIOANS MOVE, EXPANDING CHOICE
Ohio HOME choice is a non-waiver program designed to assist persons of all ages with disabilities to move from a long-term facility into home and community based services.
Requirements include:
- Residing in inpatient facility for at least 90 days
- Meet institutional level of care
- Have a Medicaid claim while in the facility
moving to a qualified residence 
A time - limited participation total of 365 actual days allows for full wrap-around services including those such as:
- Independent living skills training
- Community transition coordination
- Care management
- Social work and nutritional counseling
- Nursing
- and more!
