Q. How do I become a Home Care Waiver consumer?
A. There is qualifying criteria individuals must meet in order to become eligible for services through the Ohio Home Care Waiver. Go to http://www.ohiohcp.org to learn about the Home Care Waiver requirements on becoming a consumer, the forms needed to apply and most current consumer information.

Q. What is the difference between Home Care Waiver and Passport?
A. Ohio Home Care is Medicaid's integrated program of home care services for medically fragile individuals under age 60. The Ohio Department of Job and Family Services (ODJFS) administer’s the Ohio Home Care Program through the Bureau of Long Term Care Services & Supports (BLTCSS). Go to http://www.ohiohcp.org for this option. Ohio's PASSPORT Medicaid waiver helps Medicaid-eligible older Ohioans (over 60) get the long-term services and supports they need to stay in their homes and is administered by Ohio’s Department of Aging. Go to http:// www.aging.ohio.gov/services/passport/ for this option.

Q. Does the Home Care Waiver help with home modifications and how much is permitted?
A. Yes. Ohio Home Care Waiver provides home modifications that are medically necessary. Under the Ohio Home Care Waiver, CareStar follows OAC Rule 5101:3-46-04 (E). Home modification services are environmental accessibility adaptations to structural elements of the interior or exterior of a consumer's home that enable the consumer to function with greater independence in the home and remain in the community. Home modification services must enable the consumer to function with greater independence, avoid institutionalization and reduce the need for human assistance. There is a $10,000 limit per calendar year.
