CareStar has more than 17 years of experience serving Medicaid populations in the home environment. For its first 7 years, CareStar provided a full array of professional and paraprofessional home healthcare services to consumers in the states of Ohio, Virginia and Indiana. During the past 10 years we have concentrated solely on Medicaid Home and Community-Based Waiver programs.

Our managers’ extensive experience includes a knowledge base broad enough to establish and maintain the best consumer care, provide the most comprehensive case management and understand State and Federal guidelines.

Case Management
Our case management is defined as a collaborative process that assesses plans, implements, coordinates, monitors and evaluates options and services to meet consumers’ community care needs. We accomplish this through communication and utilization of available resources to promote quality and cost-effective outcomes.
