Civil Liberties Union of Ohio is headquartered at the Max Wohl Civil
Liberties Center in midtown Cleveland, supported by a staff of 16,
as well as numerous interns, law clerks and volunteers.
The building was purchased and renovated in 2001, and now provides
a comfortable home with the flexible space and technology that our
staff needs to support our leaders, volunteers and activists across
the state.
The ACLU has chapters and working groups throughout Ohio to promote

The Assisted
Living Waiver Program is an alternative for Nursing Facility Residents
and recipients of one of Ohios Waiver Medicaid Programs. The
purpose of the waiver is to provide an additional option on the
continuum of care for individuals who meet the nursing home level
of care criteria and whose needs can be adequately and safely met
in a less restrictive setting
PROGRAM ADMISSION/QUALIFICATION
REQUIREMENTS
In
order to be eligible for the Assisted Living Waiver the individual
must meet one of the following criteria:
-
Be a nursing facility
(NF) resident seeking admission to a residential care facility
that would remain in an NF for long term care if not for the
waiver.
-
Be
a participant on one of Ohios Medicaid Waiver Programs who would
move to a NF if not for the Assisted Living Waiver.
In addition
to one of the above, the participant must have a documented need
for hands on assistance with at least two activities of daily living
and also 21 years of age or older. |

Crossroads
is a creative and stimulating children's behavioral health organization
that offers an increasingly integrated and comprehensive system
of services for children, adolescents, and families.
The
healthy development of a child as a "whole" person in
the context of one's family, culture, care-givers, and community
is the foundation of Crossroads' service philosophy. We strive to
empower greater resiliency in children and to build protective community-based
structures and relationships; and this principle is woven into all
of our prevention, early intervention, and treatment services.
Crossroads'
service offerings include: early childhood programs, Early Head
Start, school-based interventions, community support and home-based
services, a therapeutic alternative school, therapeutic foster and
respite care, and child psychiatry. We have collaborative partnerships
with the early childhood community, child care centers, schools,
Juvenile Court, the Lake County Department of Jobs and Family Services,
and other child-serving organizations.

addition, Crossroads is a customer and performance-focused learning
