The Assisted
Living Waiver Program is an alternative for Nursing Facility Residents
and recipients of one of Ohio’s Waiver Medicaid Programs.
The purpose of the waiver is to provide an additional option on
the continuum of care for individuals who meet the nursing home
level of care criteria and whose needs can be adequately and safely
met in a less restrictive setting
PROGRAM ADMISSION/QUALIFICATION
REQUIREMENTS
In
order to be eligible for the Assisted Living Waiver the individual
must meet one of the following criteria:
-
Be a nursing facility
(NF) resident seeking admission to a residential care facility
that would remain in an NF for long term care if not for the
waiver.
-
Be a participant on one
of Ohio's Medicaid Waiver Programs who would move to a NF if
not for the Assisted Living Waiver.
In addition
to one of the above, the participant must have a documented need
for hands on assistance with at least two activities of daily living
and also 21 years of age or older. |

Our
Mission
Erie
County Care Facility will continue the tradition of excellent service
to our community. We will respect and recognize each resident as
an individual. Our professional staff will provide a quality environment
for our residents and their families. We will value every employee
for his or her unique and important contribution to our team. Each
member of our team will take a personal responsibility to enhance
the lives of those entrusted to us.
Services
24-Hour Nursing Care- Dedicated
and caring staff of Registered Nurses, Licensed Practical Nurses,
and State Tested Nursing Assistants providing nursing care 24 hours
per day, seven days per week.
Secured Alzheimer’s Unit-
A unit for residents who may wander and need to be in a secured
setting for their safety.
Skilled Services- Physical,
occupational, and speech therapies offered seven days a week.
Dietary Services- All meals
are provided by our dietary department and planned by our Licensed
Dietitian. Special diets and individual preferences are accommodated.
Ancillary Services- We offer
in-house audiology, optometry, podiatry, x-ray, laboratory, dentistry
and pharmaceutical services.
Social Services Department-
Our Licensed Social Workers can help residents and families who
are coping with the changes in their lives.
Activities Department - The
activities department coordinates activities in-house and outside
such as: shopping, movies, dining out, picnics, and special outings.
Laundry, Housekeeping, and
Maintenance Departments- Our laundry, maintenance, and housekeeping
staffs keep the residents' environment and personal belongings neat,
clean, and in good repair.
Business Offices- Any financial
concerns can be handled by our competent and courteous staff. Office
hours are 8:00 a.m. to 5:00 p.m.
Beauty Services- Our facility
offers beauty and barbershop services in our newly remodeled beauty
shop. Contact Activities for appointments.
Alzheimer’s/Dementia Support
Group -A support group is available that provides emotional support
to residents and families who may need help with the changes in
their lives. Contact Lynda Ramsey, LSW, or Jennifer Sherer, ADC
at 419-627-8733 for specific times and dates.
Hospice Services -We offer
residents and their families added support, care, and comfort.
Wound Care Specialist -We
have on staff one of the areas only Certified Wound Care Specialists.
Respite Care -Short stays
designed to relieve caregivers.
