The program is designed to serve the severely mentally disabled individuals  that have been judged incompetent by the Probate Court. 
The program offers guardianship of the person only, and will not offer guardianship of the estate. This program will however assist in the facilitation of a payee or guardian of estate when necessary.

Who is eligible for this program?

To qualify for the program, a person must be the head of a household with at least one child and temporarily homeless due to:

  • Unsafe/unsanitary living conditions
  • Natural disaster (fire, flood, etc.)
  • Lack of financial resources
  • Domestic Violence
  • Lack of utilities
  • Eviction
  • Transience
  • Condemned dwelling/displaced by the city

How do I get a referral?

Eligible families must be referred by a social service agency, such as the American Red Cross. Other acceptable agencies include:

  • Family Service Association
  • YWCA Shelter Network
  • Lutheran & Catholic Social Services
  • Montgomery County Children’s Services
  • St. Vincent de Paul Society
  • Project Cure
  • Montgomery County Job and Family Service Center
  • Ombudsman’s Office

To obtain a referral, the head of the household may request a confidential assessment of the family’s housing situation from one of the above-mentioned agencies.

Temporary Residency

Each EHP resident is provided with a maximum of 60 days of housing, clothing and food. During this time, a trained social worker is assigned to each family to assist in resolving the situation that led to their homelessness.

Referral and on-site services include:

  • Budgeting classes
  • Nutrition classes
  • Parenting classes
  • Fair housing classes
  • Housekeeping classes
  • Weekly case management
  • Transportation to appointments
  • Assistance with locating permanent housing
  • After-school tutoring program

The EHP also offers follow-up casework for up to one year and assists families in locating permanent low-cost housing.

For the Children

Educational programs are offered to children living at the EHP. Through a partnership with Dayton Public Schools, children have the opportunity to receive tutoring in their studies.

During the summer, children can participate in the Summer Reading Program. They can check out library books from selected Dayton Public Schools and participate in crafts and learning games.

In-Kind Contributions

To donate furniture, household items such as blankets, towels or sheets, or personal hygiene items such as soap, toothpaste or diapers, call 937-223-2985.

The 30 Senior Dining Centers within Montgomery County, serve hot, nutritious "home style" lunch meals to individuals 60 years of age and over. The Senior Dining Center in your neighborhood is attended by people like you, folks who enjoy going out for a low-cost but healthful and tasty lunch and who enjoy the company of their neighbors.

At the Dining Centers you can meet new people, attend picnics, and enjoy holiday celebrations. Exercise classes and entertainment are available at select Dining Centers. The Senior Dining Centers are brought to you by The Senior Resource Connection and are funded through the Area Agency on Aging Title III program, United Way and customer contributions. Many groups contribute as host facilities and special funders.

You also have the opportunity to learn more about maintaining your health and nutrition and about resources available to seniors in our community. Screening tests are brought directly to various Dining Centers throughout the year.

What Is the food like?

Hot, nutritious and delicious meals are served for both regular and diabetic needs.  Milk and bread accompany every meal.

How much does it cost?

Customers contribute according to their abilities.  A contribution of $2.00 is suggested.  Friends and family are welcome, but everyone under age 60 (except spouses) must pay $4.65 for their meals.  Ask about our volunteer program for those under 60 years of age and needing a donation-meal only.

How do I start?

You can order your lunch in advance while at your Dining Center, or you can call the number listed for the Dining Center you wish to attend by 12:30 p.m. the day before you go.

What if I don't drive?

In Montgomery County the majority of dining centers are located in the city of Dayton and are on public transportation routes.  For specific information regarding transportation in all counties, please call the individual dining center.

What if I can't get out?

Most people prefer to socialize among their peers whenever they can, but sometimes that's impossible.  People who are homebound (unable to cook or shop for themselves) and have no source of meal preparation may be eligible to receive Meals-On-Wheels.  Call the Senior Resource Connection Meals-On-Wheels at 937-228-FOOD (3663) or toll free at 1-888-580-3663. 

What happens if the weather is bad?

Major television and radio stations will announce Senior Resource Connection Dining Center closings.

The Assisted Living Waiver Program is an alternative for Nursing Facility Residents and recipients of one of Ohio’s Waiver Medicaid Programs.  The purpose of the waiver is to provide an additional option on the continuum of care for individuals who meet the nursing home level of care criteria and whose needs can be adequately and safely met in a less restrictive setting

PROGRAM ADMISSION/QUALIFICATION REQUIREMENTS

In order to be eligible for the Assisted Living Waiver the individual must meet one of the following criteria:

  • Be a nursing facility (NF) resident seeking admission to a residential care facility that would remain in an NF for long term care if not for the waiver.

  • Be a participant on one of Ohio’s Medicaid Waiver Programs who would move to a NF if not for the Assisted Living Waiver.

 

In addition to one of the above, the participant must have a documented need for hands on assistance with at least two activities of daily living and also 21 years of age or older.
