The Community
Health Foundation is organized and will operate for the exclusive
purpose of engaging in charitable, educational and scientific activities
to specifically enhance the quality of life for members of the communities
of Stark, Wayne and Tuscarawas Counties in Ohio, by educating, promoting
and supporting the health of members of those communities. Its assets
shall be devoted to initiatives and programs that promote physical,

Emergency Prescreening and
24-Hour Crisis Intervention
Evaluation and prescreening provides 24-hour telephone consultation
and referral to professional staff who are available to provide mobile
outreach assessment as well as walk-in support and linkage to other
needed services. Crisis Stabilization
These services help individuals who do not require psychiatric hospitalization,
but require 24 hour non-medical supervision. Our 5-bed unit and around
the clock staff offer support and supervision for individuals in crisis.
Case Management
Trained professionals provide case management services for children
and adults. These professionals advocate for and link clients with
community resources. |

Overview
The Society for Equal Access Independent Living Center, Inc. has
been in operation since April 1990. S.E.A. is a consumer-controlled,
cross-disability, private, non-profit organization operated by and
for individuals with disabilities. S.E.A. currently serves residents
of Belmont, Carroll, Coshocton, Guernsey, Harrison, Holmes, Jefferson
and Tuscarawas counties in Eastern-Central Ohio.
The Society
for Equal Access has two purposes:

Help
individuals with disabilities live independently.

Serve
the community by helping create an environment that is accessible
for everyone.
These goals
are accomplished through the following:

of direct services to individuals with disabilities.

Provision
of services to the community.
The Society
for Equal Access has an affirmative action plan in operation which
is supported by outreach efforts. Individuals with disabilities
are eligible to become participants without regard to race, color,
national origin, gender, age, educational attainment, or disability.
Participants set and work toward fulfillment of self-directed independent
living goals.
The
provision of independent living services empowers individuals with
disabilities, increasing the ability of these individuals to obtain
needed information and services to enhance the opportunity for greater
levels of productivity, in turn, resulting in higher levels of integration
and inclusion in the mainstream of society. |

Purpose
Our goal is to promote
a child’s wellness by monitoring the child’s growth and development
to detect any abnormal findings early. To note any signs of illness
that might require a referral, keep the child’s immunizations
up-to-date. And to encourage parents to enjoy and anticipate their
child's development through all their stages.

Minimum fees for physicals
and well-child exams depending on services provided. Please call
for fees. Medicaid, Medicare and Insurance Accepted. No one denied
care due to his or her inability to pay.
