Improving Outcomes of the Underserved (IOU)

Improving Outcomes of the Underserved (IOU) is a home health screening pilot program aimed at enhancing the early identification of diabetes or other chronic diseases among populations with limited access to healthcare services. This initiative utilizes a wrap-around model to connect individuals with essential healthcare resources, ensuring comprehensive support for the early detection and effective management of diabetes and other chronic conditions.

In Appalachian Regions of Ohio, geographic, economic, and social factors contribute to a shortage of healthcare providers. Consequently, residents often face significant barriers to accessing timely care, leading to delayed diagnoses, inadequate preventive measures, and poorer overall health outcomes.

The IOU Program addresses these challenges by assigning a Community Health Worker (CHW) as a primary point of contact. The CHW provides personalized guidance through the healthcare system, assesses individual needs, provides a health screening kit to assist with early detection of diseases or ongoing disease management and  works with individuals to develop a tailored Care Plan to support overall health.

By taking steps toward your better health you can Improve Your Life, and it is our mission to assist you in this journey. You can also receive rewards for each of the below steps you take:

  • Receive a $25 gift card for the completion of an assessment and A1C reading (Diabetes diagnosis not required).
  • Receive a $25 gift card for agreeing to ongoing care coordination including scheduled contact calls and visits.
  • Receive a $25 gift card for the completion of a second A1C test at completion of care coordination.

We would love to connect with you today! Please see below the different ways to reach out!
